  • Patient is a 35-year-old female with a history of 2 vaginal births (Gravida 2, Para 2), 5 years ago and 8 months ago. 
  • Urinary leakage - Stress Urinary Incontinence with Coughing, Sneezing, and Jumping.
  • “I have tried EVERYTHING and can’t seem to figure out how to do a Kegel. Am I even doing it right at all? I do home programs and have seen another pelvic PT - they just tell me to keep trying.”

We explore possible reasons why the patient might not be able to feel her pelvic floor contract and describe a potential reason that is sometimes missed. We further explore how to possibly cue for this and improve function with this knowledge.

  • Patient is a 50-year-old female with sitting pain at the “sit bone” area on the R side and some on the L. She is concerned that she has pudendal nerve entrapment and wants to get testing for it.
  • No known injury. Seems to be getting worse over the years, and sometimes also feels it on the vulvar tissue/perineum. 
  • She is extremely fearful that she has pudendal nerve entrapment.

Dr. Tracy Sher, PT, DPT, CSCS, has 24 years of clinical experience in pelvic health. She has dedicated her career to treating patients with complex presentations with pelvic pain and genital pain, postpartum conditions, sexual dysfunction, and pelvic health conditions affected by menopausal changes. She has also been a leading pelvic health expert and instructor internationally for 14 years - teaching and mentoring thousands in-person and online.

Dr. Sher is CEO of Pelvic Global and Founder of the Global Pelvic Health Alliance Membership (GPHAM). She also has a private pelvic health PT practice, Sher Pelvic Health and Healing, in Orlando, FL.
